ABSTRACT

Introdução: O abdome é um segmento estético-funcional importante na caracterização do contorno corporal. A confecção da plicatura do reto abdominal pode ocasionar diversos transtornos como o aumento da pressão intra-abdominal e torácica. Assim, este estudo tem como objetivo avaliar os parâmetros de pressão abdominal e pulmonar em pacientes submetidos à abdominoplastia com lipoaspiração. Métodos: Estudo longitudinal prospectivo descritivo. As pacientes foram submetidas à lipoabdominoplastia com lipoaspiração de flancos pelo mesmo cirurgião plástico sênior. A mensuração da pressão intra-abdominal foi feita por meio do uso de um dispositivo acoplado à sonda vesical de demora do paciente e a pressão intratorácica foi aferida por meio do monitor do aparelho de ventilação. As medições foram feitas antes e após a plicatura e durante a posição de Fowler. Resultados: O tempo médio do ato operatório foi de 4 horas e 8 minutos. A pressão intra-abdominal antes e após a plicatura do músculo reto abdominal variou de 2 a 11 cm H2O e 5 a 16 cm H2O, respectivamente. A pressão intratorácica antes e após a plicatura do músculo reto abdominal variou de 13 a 17 cm H2O e 14 a 18 cm H2O, respectivamente. A pressão intra-abdominal em posição de Fowler variou de 6 a 23 cm H2O. A pressão intratorácica em posição de Fowler variou de 15 a 19 cm H2O. Foi observado um caso de deiscência, um caso de seroma e um caso de desconforto respiratório. Conclusão: Os resultados elucidam que a plicatura do reto abdominal ocasiona o aumento da pressão intra-abdominal e pulmonar sem gerar alterações sistêmicas significativas.


Introduction: The abdomen is an important aesthetic and functional segment in the characterization of body contouring. The plication of the rectus abdominis may cause various problems to the patient such as increased intra-abdominal and thoracic pressure. Thus, this study aims to evaluate abdominal pressure and pulmonary parameters in patients undergoing abdominoplasty with liposuction. Methods: This is a descriptive longitudinal prospective study. All patients underwent the lipoabdominoplasty with liposuction of flanks by the same senior plastic surgeon. The intra-abdominal pressure was measured by a device attached to indwelling urinary catheter of the patient and intra-thoracic pressure was measured by the ventilation device monitor. Measurements were made before and after plication and in Fowler's position. Results: Mean surgery time was 4 hours and 08 minutes. Intra-abdominal pressure before and after plication of the rectus abdominis ranged from 2 to 11 cm H2O and 5-16 cm H2O, respectively. Intra-thoracic pressure before and after plication of the rectus abdominis ranged from 13 to 17 cm H2O and 14-18 cm H2O respectively. The intra-abdominal pressure in Fowler's position ranged from 6 to 23 cm H2O. The intra-thoracic pressure in Fowler's position ranged from 15 to 19 cm H2O. One case of small dehiscence, one case of seroma and one case of respiratory distress were observed. Conclusion: The results demonstrate that plication of the rectus abdominis increases intra-abdominal and pulmonary pressure without generating significant systemic changes.

ABSTRACT

PURPOSE: To evaluate the effects of isoxsuprine and nicotine on TRAM. METHODS: Forty eight 48 Wistar rats distributed into four Groups (n=12). All rats received medication managed daily for 20 days: saline solution (SA), nicotine solution (NI), isoxsuprine solution (IS) and nicotine solution (NI) + isoxsuprine solution (IS). On day 21st the rats were submitted to the caudally based, right unipedicled TRAM flap and after 48 hours, made the macroscopic evaluation of the surface of the flap, photographic documentation and collection of material for histology. Data from macroscopic evaluation were analyzed by ANOVA and microscopic evaluation by Kruskal-Wallis test, with significance level of 5%. RESULTS: In the macroscopic evaluation of isoxsuprine Group retail presented absolute numbers: final area (p=0.001*) and viable area (p=0.006*) with the highest values; necrosis (p=0.001*) had the lowest value. Microscopic examination revealed no significant findings in the study of TRAM under the action of isoxsuprine and nicotine to the percentage of necrosis in the left and right cranial and caudal regions. CONCLUSIONS: There was significant improvement in viability of TRAM using the isoxsuprine solution alone. No influence using nicotine alone and in association with isoxsuprine. .

ABSTRACT

Introdução: A correção da diástase dos músculos retos do abdome é realizada principalmente pela sua plicatura, procedimento que consome muito tempo cirúrgico. O objetivo deste estudo é avaliar a eficácia e o tempo necessário para a correção da diástase dos retos do abdome comparando a plicatura com sutura contínua em plano único à plicatura em dois planos. Método: Foram incluídas 20 mulheres com história de pelo menos uma gestação. Foram randomizadas em dois grupos, um submetido à plicatura dos retos do abdome em dois planos (controle) e outro à sutura em plano único contínuo (estudo) com mononylon 2-0. Mediu-se o tempo necessário para a realização de cada uma das técnicas. Para análise estatística, foram usados os testes não paramétricos de Mann-Whitney, Friedman e Wilcoxon, considerando estatisticamente significante p < 0,05. Todas as pacientes foram submetidas à ultrassonografia previamente à cirurgia, 3 semanas e 6 meses de pós-operatório. Resultados: A plicatura do grupo estudo mostrou-se tão eficaz quanto à do grupo controle, mantendo seu resultado em longo prazo, apresentando diferença estatisticamente significante (p = 0,018) na comparação das medidas pré e pós-operatórias. No grupo estudo, porém, a plicatura consumiu menor tempo cirúrgico para ser realizada, também com diferença estatisticamente significante (p = 0,002). Conclusão: Com este estudo, pode ser concluído que tanto a sutura em dois planos como a em plano único contínuo são eficazes na correção da diástase dos retos do abdome e mantêm o resultado por longo tempo. Concluiu-se também que a técnica de sutura contínua demanda menor tempo para ser realizada.


Introduction: The correction of diastasis of the rectus abdominis muscles is primarily performed using the plication technique, which involves a surgical procedure. To evaluate the efficacy and the surgical time necessary to correct diastasis of the rectus abdominis muscles using plication and sutures in one continuous or two planes. Method: Twenty women with at least one pregnancy were included in the study. The study participants were randomized into two groups: the control group was subjected to plication in two planes, and the experimental group was subjected to plication using suture in one continuous plane with mono-nylon 2-0. The time required to perform each of these techniques was measured. For statistical analysis, the nonparametric Mann-Whitney test, Friedman test, and Wilcoxon test were used, and p-values of < 0.05 were considered statistically significant. All patients underwent ultrasonography before surgery, and at 3 weeks and 6 months postoperatively. Results: The plication technique was similarly effective in the two groups, achieving a long-term surgical outcome; however, the differences in the ultrasound measurements in the pre- and postoperative periods were statistically significant (p = 0.018). In addition, plication was performed in a significantly shorter time in the experimental group (p = 0.002). Conclusion: Sutures in one continuous plane and in two planes were effective in correcting diastasis of the rectus abdominis muscles, with the maintenance of a long-term surgical outcome. In addition, the continuous suture technique can be performed in a shorter time.

ABSTRACT

La endometriosis se describe como la presencia de tejido endometrial funcional fuera de la cavidad uterina. La pared abdominal es un sitio raro de endometriosis extrapélvica, la cual se desarrolla usualmente a partir de cicatrices por procedimientos quirúrgicos. La endometriosis del músculo recto del abdomen es una ocurrencia excepcional y solamente se han descrito 21 casos en la literatura médica. PRESENTACIÓN DECASO: Mujer de 42 años de edad, con antecedentes de cesárea de hace 4 años, es ingresada al Hospital Alemán-Nicaragüense, Nicaragua, en agosto del 2013 por presentar masa dolorosa en región mesogástrica. Esta tumoración presentaba aumentos cíclicos de tamaño y de dolor que coincidían con la fase folicular del período menstrual. Los exámenes ecográficos sugerían una hernia incisional. La laparotomía reveló la presencia de una masa de53 cm2 adherida al musculo recto del abdomen y a tejido conectivo circundante. Los cortes histológicos mostraron glándulas y estroma endometrial integradas en una masa fibrosa densa por lo que se hace el diagnóstico de endometrioma. DISCUSIÓN: La endometriosis de pared abdominal es una entidad clínica que se caracteriza por la presencia de una tumoración localizada cerca o dentro de la cicatriz quirúrgica. La ausencia de síntomas clásicos y el sitio inusual de la lesión hacen que el diagnóstico de esta entidad clínica sea un poco complicado. El tratamiento de elección es quirúrgico. Endometriosis is described as the presence of functioning endometrial tissue outside of the uterine cavity. The abdominal wall is an uncommon site of extrapelvic endometriosis, which usually develops in association with a prior surgical scar. The rectus abdominis muscle endometriosis is an exceptional occurrence and only 21 cases have been reported in the medical literature. CASE REPORT: A 42 year-old woman who underwent caesarean section 4 years ago is admitted to the¨Hospital Aleman-Nicaraguense¨, Nicaragua, in August, 2013because of a painful mass in the mesogastric region. The pain and swelling gradually increased in a cyclical manner and aggravated during the follicular stage of the menstrual cycle. The radiological imaging revealed an incisional hernia. Laparotomy revealed a 53 cm2 mass attached to the rectus abdominis muscle and to the connective tissue. Microscopy showed glandular spaces and endometrial stroma between a dense fibrous mass, therefore endometriosis diagnosis was made. DISCUSSION: Abdominal wall endometriosis is a clinical entity characterized by the presence of a tumor located somewhere near or inside a surgical scar. Lack of the classical symptoms and the unusual site can make diagnosis difficult. The treatment of choice is the complete surgical excision. ABSTRACT

OBJECTIVE: to evaluate the role of fibrillar extracellular matrix components in the pathogenesis of inguinal hernias. METHODS: samples of the transverse fascia and of the anterior sheath of the rectus abdominis muscle were collected from 40 men aged between 20 and 60 years with type II and IIIA Nyhus inguinal hernia and from 10 fresh male cadavers (controls) without hernia in the same age range. The staining technique was immunohistochemistry for collagen I, collagen III and elastic fibers; quantification of fibrillar components was performed with an image analysis processing software. RESULTS: no statistically significant differences were found in the amount of elastic fibers, collagen I and collagen III, and the ratio of collagen I / III among patients with inguinal hernia when compared with subjects without hernia. CONCLUSION: the amount of fibrillar extracellular matrix components did not change in patients with and without inguinal hernia. .


OBJETIVO: avaliar a participação dos componentes fibrilares da matriz extracelular na etiopatogenia das hérnias inguinais. MÉTODOS: foram retiradas amostras da fáscia transversal e da bainha anterior do músculo reto abdominal de 40 homens na faixa etária entre 20 e 60 anos, portadores de hérnia inguinal tipo II e IIIA de Nyhus e de 10 controles constituído por cadáveres frescos, na mesma faixa etária, sem hérnia. A técnica de coloração foi a imunoistoquímica para colágeno I, colágeno III e fibras elásticas e a quantificação dos componentes fibrilares foi realizada através de sistema computadorizado e software de processamento e análise de imagem. RESULTADOS: não foram encontradas diferenças estatisticamente significantes na quantidade de fibras elásticas, colágeno I e colágeno III, e na proporção de colágeno I/III entre os pacientes portadores de hérnia inguinal em comparação com indivíduos sem hérnia. CONCLUSÃO: a quantidade dos componentes fibrilares da matriz extracelular não se alterou nos pacientes com e sem hérnia inguinal. .

ABSTRACT

PURPOSE: To investigate the blood vessels' concentration in TRAM flap's rat model, in the presence of pentoxifylline. METHODS: 32 male, Wistar-EPM rats were divided into two groups. Control group (C): 0.5 ml of saline, intraperitoneally, once a day, for seven days before flap elevation; PTX group (P): pentoxifylline (20mg/kg/day), intraperitoneally, for seven days before flap elevation. After that, they were submitted to a caudal unipedicle TRAM flap. On the fifth postoperative day, percentages of flap necrosis were determined via the "paper template" method and Tram flap's zone IV skin biopsies were taken for histological analysis. RESULTS: the mean percentage of flap necrosis in group C was 58.7 % and in group P, 31.1 (Wilcoxon test; p = 0.003). Mean capillary vessels number in zone IV's skin in C group was 33.4 and in P group was 71.9 (p=0.008). CONCLUSIONS: Pentoxifylline was effective reducing the necrosis in the caudal unipedicle TRAM flap in the rat as well as increasing the number of capillaries in an ischemic zone (zone IV). .

ABSTRACT

PURPOSE: To investigate the microscopic changes in the rectus abdominis muscle in rats subjected to five delay procedure. METHODS: 30 male holtzmann rats, weighting between 250 and 350 grams, were used. The animals were divided into five groups (n=6): A - cranial section of the right muscle; B - caudal section of the right muscle; C - craniocaudal section of the right muscle; D - cranial section reflecting the right muscle in the craniocaudal direction; E - caudal section reflecting the right muscle in the caudocranial direction. On the seventh day after surgery, a resection of the cranial and caudal fragments of the right and left muscles, respectively, was performed for microscopic analysis. Histological alterations were quantified and the right and left (control) muscle fragments compared. Fisher's exact test was used for statistical purposes with a significance level of 5 percent. RESULTS: The comparison between right and left muscles showed statistically significant differences in group A - inflammatory infiltrate in the cranial fragment (p=0.015); in group C - inflammatory infiltrate (p=0.000) and necrosis (p=0.015) in the caudal fragment; and in group E - edema in the caudal fragment (p=0.000). No significant alterations were noted in groups B and D. CONCLUSIONS: Irrigation exclusively through the perforating muscle vessels is inappropriate; irrigation exclusively through the cranial pedicle causes milder, insignificant histological alterations, when compared with irrigation exclusively through the caudal pedicle; irrigation exclusively through the caudal pedicle causes more pronounced, but still insignificant, histological alterations when compared with other forms of devascularization.


OBJETIVO: Investigar as alterações microscópicas no músculo reto do abdome de ratos submetidos a cinco técnicas de autonomização. MÉTODOS: Utilizaram-se 30 ratos machos Holtzmann, pesando entre 250 e 350 gramas. Os animais foram distribuídos em cinco grupos (n=6): A - secção cranial do músculo direito; B - secção caudal do músculo direito; C - secção cranial e caudal do músculo direito; D - secção cranial rebatendo o músculo direito no sentido cranial-caudal; E - secção caudal rebatendo o músculo direito no sentido caudal-cranial. No sétimo dia pós-operatório, ressecou-se um fragmento cranial e caudal dos músculos direito e esquerdo, respectivamente, para estudo microscópico. As alterações histológicas foram quantificadas e comparados os fragmentos dos músculos direito aos do esquerdo (controle). Utilizou-se, para fins estatísticos, o teste exato de Fisher, com nível de significância de 5 por cento. RESULTADOS: A comparação entre os músculos direito e esquerdo demonstrou significância estatística no grupo A - infiltrado inflamatório no fragmento cranial (p=0,015); no grupo C - infiltrado inflamatório (p=0,000) e necrose (p=0,015) no fragmento caudal; e no grupo E - edema no fragmento caudal (p=0,000). Não foram observadas alterações significativas nos grupos B e D. CONCLUSÕES: A irrigação exclusiva pelos vasos musculares perfurantes é inadequada; a irrigação exclusiva pelo pedículo cranial causa alterações histológicas menos acentuadas, não-significantes, em comparação à irrigação exclusiva pelo pedículo caudal; a irrigação exclusiva pelo pedículo caudal causa alterações histológicas mais acentuadas, não-significantes, em comparação com as demais formas de desvascularização.

ABSTRACT

PURPOSE: To assess the collagen content and types in the rectus abdominis muscle of cadavers of different ages. METHODS: Forty fresh adult male cadavers, at room temperature, were obtained from the Institute of Legal Medicine of Franca and dissected within 24 hours of death. The cadavers were divided into two groups: Group A (n=20), 18 to 30 years of age, and Group B (n=20), 31 to 60 years of age. Bilateral incisions were made in the middle portion of anterior rectus sheath 3 cm superiorly and 2 cm inferiorly to the umbilicus and four fragments of the rectus abdominis muscle were dissected. The samples were fixed in 10 percent buffered formalin and sent for immunohistochemical analysis to determine collagen content and types. RESULTS: Immunohistochemical results revealed higher amounts of type I and type III collagen in Group A. However, no difference in the amount of type IV collagen was found between the groups. CONCLUSION: The amount of type I and type III collagen was higher in group A.


OBJETIVO: Avaliar o colágeno no músculo reto do abdome em cadáveres de diferentes faixas etárias. MÉTODOS: Foram dissecados 40 cadáveres adultos masculinos, não fixados, com tempo de óbito de até 24 horas, em temperatura ambiente, provenientes do Instituto Médico-Legal de Franca (SP - Brasil). Os cadáveres foram distribuídos em dois grupos: GRUPO A (n=20) - 18 a 30 anos e GRUPO B (n=20) -31 a 60 anos. Realizou-se incisão na porção mediana da lâmina anterior da bainha do músculo reto a 3 cm superiormente e 2 cm inferiormente ao umbigo em ambos os lados, sendo retirados quatro fragmentos de músculo reto do abdome. Esse material foi conservado em formalina tamponado a 10 por cento e enviado para imuno-histoquímica para determinação do tipo de colágeno. RESULTADOS: Na Imunihistoquímica os colágenos I e III foram estatisticamente maiores no grupo A, porém não houve diferença entre os grupos em relação ao colágeno IV. CONCLUSÃO: A quantidade de colágeno tipo I e III foi maior no grupo A.

ABSTRACT

El objetivo de este trabajo es valorar la incidencia relativa de hematomas abdóminopelvianos no sospechados en tomografía computada (TC) y revisar las causas más frecuentes, haciendo una puesta al día de cada patología en particular, se analizaron retrospectivamente 1.813 TC de abdomen y/o pelvis realizadas en nuestro Servicio entre enero de 1996 y julio de 1997 en pacientes explorados por diversas causas. Seis fueron los casos positivos. Fueron excluidos los pacientes con sospecha clínica inicial de la presencia de hematomas y los portadores de patología traumática y los casos de hematomas secundarios a complicaciones aneurismáticas aortoilíacas. La TC demostró ser un método sensible y específico en el diagnóstico de este grupo de patologías, aún en ausencia de sospecha clínica inicial. Consideramos esencial el conocimiento de las características de los hematomas del recto anterior del abdomen por su incidencia, sus potenciales complicaciones y su cuadro clínico poco específico
